Where it came from

Mr. Burns stares at a workplace sign reading "[X] days without [something happening]", and the number is always zero. You label the sign with whatever thing you can't go a single day without: a weird thought, a specific mistake, a recurring frustration. The joke is that you were never going to make it past zero anyway.
The workplace-safety framing gives the meme a very specific "tracking a problem that nobody can actually fix" flavor. It's not about failure exactly — it's about inevitability. Of course it's zero. What did we expect.
